A Home Care Plan Checklist for Families
A clear care plan helps family members and caregivers share the same expectations. Use this checklist to record routines, preferences, tasks, and communication needs.
A written home care plan is a practical guide to the person's routines and agreed support. It should be easy to update and should not include more private information than caregivers need. Use it to align family expectations with the tasks the provider has agreed to perform.
Include everyday preferences
- Preferred name, communication style, and language needs.
- Usual wake, meal, rest, and activity routines.
- Food preferences, household routines, and meaningful activities.
- Mobility considerations and any instructions provided by a clinician.
- Tasks the person wants to continue doing independently.
Define the caregiver's role
List each agreed task and when it should happen. Be specific about what is not included. For non-medical care, do not assume the caregiver can provide nursing services, make clinical decisions, or change a treatment plan. Ask the provider to explain its boundaries and how to handle a concern that requires a clinician.
Keep the plan usable: use clear words, identify the most important tasks first, and date each revision. Avoid instructions that conflict with a clinician's directions or ask a caregiver to make a judgment outside the service agreement. If a task becomes more difficult or unsafe, pause and ask the provider and appropriate professional how to proceed.
Set communication and escalation steps
Name the family contact, the preferred way to send routine updates, and who should be contacted if a scheduled visit cannot happen. Record emergency contacts and the family's chosen response plan. Keep the plan current when phone numbers, schedules, or care needs change.
Review it together
Ask the older adult, family contact, and provider to review the plan before care begins. Revisit it after the first week and whenever a meaningful change occurs. A care plan should guide a respectful working relationship, not become a substitute for listening to the person receiving care.
